Patients may be referred for consultation, evaluation, and treatment.

New Patient Requirements:

• A written referral
• All medical records pertaining to the present spine problem including physicians' note, X-rays, MRI's, CT Scans, etc.
• Any notes from other physicians or healthcare professionals who have been involved in the management of the pain (e.g. surgeons, pain management specialists, neurologists, etc.)

We participate with most health insurance plans including Medicare.
